- Original Form 137/SF10-JHS (Learner’s Permanent Academic Record)
- PSA-issued Birth Certificate
- Original Copy of Certificate of Good Moral Character or Recommendation from the School Principal
- Medical Certificate of Chest X-ray results
- Medical Certificate of Hepatitis A & B Screening (for applicants in Culinary Arts or Restaurant and Café Operations)
Senior High School Transferee (Incoming Grade 12)
- Certificate of Transfer (Honorable Dismissal)
