  1. Original Form 138 / SF9-JHS (Learner's Progress Report Card)
  2. Original Form 137 / SF10-JHS (Learner's Permanent Academic Record)
  3. PSA-issued Birth Certificate

Senior High School Transferee (Incoming Grade 12)

  1. Certificate of Transfer (Honorable Dismissal)
  2. Original Form 138 / SF9-SHS (Learner's Progress Report Card)
  3. Original Form 137 / SF10-SHS (Learner's Permanent Academic Record)
  4. PSA-Issued Birth Certificate


College



College Freshmen

  1. Original Form 138 / SF9-SHS (Learner's Progress Report Card)

College Transferees
